Health Requirements for Enrollment
PLEASE NOTE: the following health documents are required to complete enrollment ONLY UPON formal admittance into a class through the application process. Membership will contact you with the appropriate due dates for all required forms once your child has been accepted.
Health form required for all STUDENTS:
Physicians Report completed by a licensed pediatrician for each enrolling child
Health information required for all PARTICIPATING ADULTS:
Proof of immunity to Measles
Proof of immunity to Pertussis
Proof of negative Tuberculosis (TB) test OR an Adult TB Risk Assessment Questionnaire and Certificate of Completion completed and signed by a healthcare provider
