Health forms and other requirements

PLEASE NOTE: the following health documents are required to complete enrollment ONLY UPON formal admittance into a class through the application process. Membership will contact you with the appropriate due dates for all required forms once your child has been accepted.

• Health form required for STUDENTS:

• Physicians Report completed by a licensed pediatrician for each enrolling child

• Health information required for all PARTICIPATING ADULTS:

• Proof of immunity to Measles

• Proof of immunity to Pertussis

• Proof of negative Tuberculosis (TB) test OR an Adult TB Risk Assessment Questionnaire and Certificate of Completion completed and signed by a healthcare provider
